Patient

  1. Correct patient identification, treatment plan, localisation, marking, consent forms, X-rays, blood results
  2. Patient dress-change into hospital clothing, washing before surgery, time, liquids used
  3. Preparation of the operating site: time taken, disinfection liquid used.
  4. Draping the operating site, securing the patient, satff, anaesthetising instrumentation such as the laryngeal mask or Guedel airway 

Equipment

  1. General instrument trays
  2. Burs, handpieces, saws and blades, plates and screws with company name, address, lot number, reference number, expiry date, signs of sterility, all data entered into the patient file
  3. Special equipment such as microscope, distractor, dermatome, ready for use, sterile, with all connections suitable for use 

Institution

  1. Sterilisation procedure: documented person, procedure (134C, 50 min., vacuum peaks reached etc.), documented surgical instrument trays, signature.
